David A. Formwalt, 59, of Catonsville, died Wednesday, January 26, 2022 at Howard County General Hospital. Born June 24, 1962 in Baltimore, he was the son of the late James G., Sr. and Shirley A. Zeltman Formwalt. David held various jobs. The most recent was an associate at Car Quest. He enjoyed sports of all kinds, especially football, Baltimore Orioles baseball and Maryland Terrapin’s men’s and women’s basketball. David is survived by sisters Karen L. Roach and husband Jim, Kim A. Bair and husband George, brother James (Jay) G. Formwalt, Jr., nieces Heather Bateman, Kristen Carter, Lauren Mason and husband Kevin, Kaitlin Deal and husband Jon, nephew Joshua Formwalt, great nieces Sulli, Alli and Lizzie Bateman, Aaliyah and Paige Mason, Eliza and Maris Carter and great nephews Brantley and Cayson Deal and Finley Carter. Funeral services and interment will be private. In lieu of flowers, sympathies may be expressed in the form of contributions to The Hoffa Foundation.org. The Jeffrey N.
